Abstract: 本研究の目的は,介護保険施設における看護職の勤務体制や職位等の状況,看護ケアの実施状況,研修の実施状況等の実態を把握し,質の高い看護ケア提供に向けた課題を明らかにすることである。大阪府内の介護保険施設の看護管理者に独自に作成した調査票を郵送し,回答の得られた236件(有効回答率38.4%)について分析を行った。その結果,ケアの実施状況として「健康状態に関するアセスメント」については看護職単独または介護職との協働で実施しており,「日常生活援助」の大半は介護職が中心となって実施し,「医療処置」「緊急時の判断」はほとんどの項目で看護職が実施していた。また研修に対するニーズは全般的に高かったが,希望する研修内容は施設の種類によって差がみられた。調査結果から,質の高い看護ケア提供に向けた課題として,介護職との連携と看護職の役割の明確化,地域や他施設との連携,研修を支援するシステムの確立が示唆された。
The purpose of this study was to understand the actual conditions of nursing staff at long-term care insurance facilities with regard to their shift systems, positions, nursing care implementation, and training in order to identify issues in providing high-quality nursing care. A questionnaire was designed and mailed to nurse administrators at long-term care insurance facilities in Osaka Prefecture, and analysis was performed for a total of 236 responses (valid response rate, 38.4%). Results for care implementation showed that "assessment of health status" was conducted by nursing staff either alone or in collaboration with care staff, "assistance with daily living activities" was mostly given with care staff playing a central role, and most items of "medical procedures" and "judgment of emergencies" were tasks performed by nurses. In addition, although training needs were generally high, they varied depending on facility type. Based on these findings, the following were indentified as issues in providing high-quality nursing care : cooperation with care staff, clarification of nurses' responsibilities, cooperation with community and other facilities, and establishment of systems for supporting training.
